Case Management
Our case management team assesses each person’s situation and needs,
then develops a plan to assist the client in obtaining essential medical, social and supportive services, such as:

• Home Health Services
• Hospice
• Housing
• Insurance
• Medicaid
• Social Security Income
• Social Security
Disability Income
• Temporary Disability Income
Client Service Funds
Financial assistance is available to eligible
clients to help obtain or maintain access to primary health care,
prescription medications not covered by insurance, nutritional support,
stable housing, and other supportive services
Treatment Information
We provide current, relevant treatment
information to clients and health care professionals throughout the year
via newsletters, workshops, and conferences.
